CSV columns (header row, any order): prompt, option_a, option_b, option_c, option_d, correct (a letter A–D or the exact correct option text). Optional: topic, body_system, cognitive_level, bloom_level, objective, explanation. QTI: single-choice multiple-choice items are detected automatically.
